Position Summary:
The Mental Health Case Manager is responsible for delivering Mental health Targeted Case Management (MH-TCM) services to adults with Serious and Persistent Mental Illness (SPMI) and children with Severe Emotional Disturbance (SED). This role involves conducting ongoing assessments of clients’ changing needs, collaborating with clients and their service providers to develop comprehensive plans, linking clients to appropriate community resources, and monitoring the effectiveness of planned services. The Case Manager’s activities are designed to facilitate access to a wide range of mental health and related services in the areas such as social, educational, vocational, and health services.
Position Outcomes:
- Under the direction of a designated clinical supervisor, provide case management services (information, plan referral and linkages) to clients on her/his assigned caseload.
- Conduct thorough assessments of clients’ mental health needs, strengths, and goals.
- Collaborate with clients and their service providers to develop individualized service plans that address identified needs
- Regularly review and update service plans to ensure they algin with clients’ evolving requirements.
- Identify and link clients to appropriate community resources, including mental health services, social services, educational programs, health services, vocational services, recreational activities, and related services.
Knowledge, Skills, and Abilities: - Knowledge of demographics, culture, customs, and geography of the White Earth Reservation.
- General knowledge of mental health targeted case management services with an understanding of local community resources and how to utilize these resources to meet client needs.
- Knowledge and understanding of the clinical work done in a community mental health program.
- Strong organization, verbal, and written communication skills.
- Skilled in identifying and resolving problems independently in a variety of settings with both clients and professionals within the community.
- Ability to manage time efficiently in order to ensure paperwork and billing are on time and completed correctly.
Qualifications: - Bachelor’s Degree in one of the behavioral sciences or related fields, including but not limited to social work, psychology, or nursing from an accredited college or university.
OR: - Without a bachelor’s degree, must:
- Have a minimum of three years of experience as a case manager associate.
OR: - Be a registered nurse and have a combination of specialized training in mental health, with three years of work experience consisting of community interaction and involvement or community discharge planning in a mental health setting.
- A valid Driver’s License and insurance for travel between sites and other business-related destinations.
- Must be able to pass a criminal background check and drug screening.
