Tribal Home Visiting Program
The home visiting program promotes family self-sufficiency and improves the health and well-being of families. Native and non-native expectant families with children birth to five living on the White Earth Reservation are eligible for home visiting services.

Program Vision:
By honoring our Anishinaabe values, celebrating family strengths, and fostering strong community ties, we will use personalized approaches to build strong, collaborative relationships that support and uplift child health and development and family well-being.

Program Goals:
(1) Home visitors will provide personalized, culturally-informed programming to our families (2) Home visitors will feel supported in addressing the social, emotional, and behavioral needs of their families (3) Our program will actively seek families that have not previously been enrolled in the program (4) Families that want to transition from LIFE/NFP to PAT will be retained

LIFE/NFP Program:
Nurse Family Partnership is an evidence-based home visiting program that helps support families. Nurses provide home visits to families living within the reservation boundaries. The LIFE nurses visit with expecting moms and families with children under 2 years old. During the home visit nurses provide education, developmental screens and immunizations. Nurse home visitors work with the family to set goals.

Parents as Teachers Program:
Parent educators serve families with children from 2 years to five years. During a home visit with the family, they cover parent-child interaction, family wellbeing, and talk about the child's development. Each visit is planned with the parents for the next tie we meet. A parent educator coached the parent on ways to interact with their child with things in their home and get them ready for kindergarten. Each month a group connection is held in a community where families come and do an activity and learn together.

Tribal Home Visiting Staff also provides the following:

  • Hearing and Vision Screenings
  • Child and Teen Check ups
  • Immunizations
  • Lactation support

Referrals can be made by calling the Tribal Health building Monday-Friday 8am-4pm or by filling out the referral form online.
