Pay: $26.23/hr. Benefits: Full Status: Full Time
The Behavioral Health Case Manager is responsible for delivering integrated case management services to adults with substance use disorder (SUD). The role encompasses a wide range of responsibilities, including engagement, assessment, planning, advocacy, coordination, and linkage of clients to formal and informal activities, services, and supports within the community. The primary objective is to promote recovery, wellness, and safety for individuals struggling with SUD. The Behavioral Health Case Manager will also provide comprehensive addiction counseling services with a strong focus on referral and service coordination, individual, group, family and crisis therapeutic interventions, service plan development and monitoring, and accurate documentation.
Position Outcomes:

  • Provides comprehensive case management focused on the holistic needs of the client and stresses comprehensive assessment, service planning, and service coordination to address multiple aspects of a client’s life.
  • Provide referrals facilitating the client’s utilization of available support systems and community resources to meet needs identified in clinical evaluation and/or treatment planning.
    Knowledge, Skills and Abilities:
  • Knowledge of ASAM Level of Care and other modalities of treatment and recovery.
  • Knowledge of 12 Core functions according to licensure/certificate.
  • Knowledge of DAANES and other data reporting standards.
  • Knowledge and skills in Person Centered-Cognitive Behavioral Theories.
    Qualifications:
  • Associates Degree (AA) in Criminal Justice, Human Services, or related filed or High School Diploma/GED with two years’ experience in related field.
  • Licensed Alcohol and Drug Counselor (LADC) or exempt from licensure under Minnesota Statues, chapter 148C, or certified as an alcohol and drug counselor under UMICAD or a person licensed as an Alcohol and Drug Counselor under licensing standards approved by Tribal Council, or its designee preferred; or ability to obtain within 6 months of hire.
  • Have Treatment Coordination certifications, or ability to obtain within 60 days of hire.
  • Ability to complete Rule 25/Comprehensive Assessment Certification within 6 months of hire.
  • Documented one-year case management experience in substance abuse, mental health, or criminal justice including culturally appropriate service delivery.
  • Two (2) years’ experience with outreach, case management or direct service with individuals with history of mental illness, homelessness, and/or substance/opioid use disorder.
  • Two (2) years or more experience working in Native American communities providing culturally appropriate services.
  • Cultural knowledge and ability to apply it to individual and group work.
  • Ability to work flexible hours/evenings and weekend, including on-call scheduling to respond to crisis after business hours and weekends.
  • Ability to work in dangerous situations, environment, and/or communities in crisis situations involving substance abuse.
  • Must be over the age 18, responsible, mature, healthy adult who is able to carry out the license holder’s duties.
  • Must pass criminal background check/applicant background study.
  • Must be free of chemical use problems for at least the two years immediately preceding hiring and must sign a statement attesting to that fact and freedom from chemical use problems must be maintained during employment.
  • Must pass drug screen at hire and throughout employment according to the White Earth HR policy.
  • Must have a Valid MN driver’s license and insurance for travel between sites and other business-related destinations.
