Our mission of the CHR/Transportation program is to improve the health and wellness of our people of the White Earth Nation through transportation, health education, social support and advocacy services.
CHR/Transportation has 3 dispatchers, 14 CHR/Transportation Aides and 1 Manager. The purpose of our program is to improve the general health status of our people of the White Earth Nation. Transportation is provided to medical facilities only. Clients must live within a 25-mile radius of the White Earth Reservation to receive services. Clients must be enrolled or descendants of White Earth or be enrolled in MA/Medicaid for reimbursements. Parents must accompany children under the age of 18. Medical appointments include dialysis, cancer treatments, cardiac, dental, medical and mental health. Clients must be seen at the closest facility that provides services. Clients must receive a referral from their primary care provider for long-distance/specialty clinic appointments.
