We provide a free visit within the home or nursing facility to help clients plan services and other needed supports that allow a client to remain in the home. Services, such as Personal Care Attendants to Life Line services are available to a client based on program eligibility.

Long-term Services and Supports Program Overviews
These publicly funded long-term programs support people with a variety of disabilities, including development disabilities, chronic medical conditions, acquired or traumatic brain injuries and physical disabilities. As often as possible, we partner with a variety of agencies that provide flexible, cost-effective home and community-based services.
White Earth Nation is a state licensed tribal lead agency. The LTSS team conducts MnCHOICES assessments to determine a person's eligibility for these programs and services. They assist with referrals for services based on a person's assessed needs.
Services LTSS programs can help provide:

Life alert phones- these are an in-home emergency response option for individuals to have the ability to call for help by pushing a button during a fall or another emergency. To be eligible for this service, people must live on the reservation and have active landline phone service.

Meal Services- there are two separate options available

Elderly Nutrition Program (ENP)- White Earth Reservation has on-site dining at multiple villages. Any elder 55+ is encouraged to enjoy a meal at any of these locations Monday through Friday.

Home-delivered meals- this requires an assessment by the LTSS team to determine eligibility.

Community First Services and Supports-this state-funded program was formerly the PCA (personal care assistance) program. As of 10/1/24, Minnesota has updated the program to offer more services, provide more flexibility, and allow people greater independence.

Waiver case management-for people who meet eligibility requirements, determined by a MnCHOICES assessment and financial eligibility. Case managers help coordinate services and supports for elderly and disabled individuals.
